چکیده

Background: Chronic obstructive pulmonary disease (COPD) is associated with cardiovascular disease. We investigated whether sleeping at altitude increases nocturnal heart rate (HR) and other markers of risk or arrhythmias in lowlanders COPD this can be prevented by oxygen therapy (NOT). Methods: Twenty-four patients, median age 66 years forced expiratory volume 1 s (FEV ) 55% predicted, living <800 m underwent sleep studies Zurich (490 m) during 2 sojourns days each St. Moritz (2,048 separated 2-week washout m. During nights 2,048 m, patients received either NOT NOT) ambient air placebo) 3 L/min via nasal cannula according to a randomized, placebo-controlled crossover trial. Sleep comprised ECG pulse oximetry measure HR, rhythm, HR-adjusted QT interval (QTc), mean saturation (SpO ). Results: In the first 490 placebo, NOT, medians (quartiles) SpO were 92% (90; 94), 86% (83; 89), 97% (95; 98) HR 73 (66; 82), 82 (71; 85), 78 bpm (67; 74) ( P < 0.05 all respective comparisons). QTc increased from 417 ms (404; 439) 426 (405; 440) placebo 0.05) was 420 = NS vs. placebo). The number extrabeats complex similar over conditions. Conclusions: While staying experienced hypoxemia association an prolongation interval. significantly improved lowered without changing QTc. Whether would reduce arrhythmia longer remains elucidated.

A randomized trial of nocturnal oxygen therapy in chronic obstructive pulmonary disease patients.

The beneficial effects of nocturnal oxygen therapy (NOT) in chronic obstructive pulmonary disease (COPD) patients with mild-to-moderate daytime hypoxaemia (arterial oxygen tension (Pa,O2) in the range 7.4-9.2 kPa (56-69 mmHg)) and exhibiting sleep-related oxygen desaturation remains controversial. The effectiveness of NOT in that category of COPD patients was studied. The end points included pu...

Chronic Obstructive Pulmonary Disease and Cardiac Repolarization: Data from a Randomized Controlled Trial.

BACKGROUND Altered cardiac repolarization is a risk factor for sudden cardiac death and seems to be increased in chronic obstructive pulmonary disease (COPD) patients. OBJECTIVE Lung volume reduction surgery (LVRS) has been shown to improve breathing mechanics and lung function in patients with severe COPD and emphysema and possibly also improve altered cardiac repolarization. METHODS Thirt...

Dexamethasone vs Hydrocortisone in Management of Exacerbated Chronic Obstructive Pulmonary Disease Patients: A Randomized Controlled Trial

Background: Different types of corticosteroids with different characteristics, such as hydrocortisone and dexamethasone, have been used in treatment of exacerbated chronic obstructive pulmonary disease but drug of choice unclear. The main goal of present study was to compare the efficacy of hydrocortisone and dexamethasone on clinical symptoms, peak expiratory flow, oxidative stress factors and...

Multi-center, randomized, placebo-controlled trial of nocturnal oxygen therapy in chronic obstructive pulmonary disease: a study protocol for the INOX trial

BACKGROUND Long-term oxygen therapy (LTOT) is the only component of the management of chronic obstructive pulmonary disease (COPD) that improves survival in patients with severe daytime hypoxemia. LTOT is usually provided by a stationary oxygen concentrator and is recommended to be used for at least 15-18 h a day. Several studies have demonstrated a deterioration in arterial blood gas pressures...

Predicting Nocturnal Hypoventilation in Hypercapnic Chronic Obstructive Pulmonary Disease Patients Undergoing Long-Term Oxygen Therapy

Background: Chronic obstructive pulmonary disease (COPD) patients are very sensitive to changes in pulmonary mechanics and central ventilation control during sleep and may develop significant gas exchange alterations with increased hypoxemia and hypercapnia. Oxygen therapy improves nocturnal desaturation but can worsen hypoventilation. Objectives: To analyze the prevalence of nocturnal hypovent...
